MEDICAL HISTORY QUESTIONNAIRE
A standardized screening medical history questionnaire will be provided to each of your employees prior to their scheduled appointment date. The employee will complete the questionnaire and return it to the technician at the time of testing. All positive responses will be printed on the health profile report.

PHYSICIAN EXAMINATION
The physical examination portion of the testing will be performed by a licensed physician. The examination is conducted in a private examining room, according to a standard protocol. The physician will inform the employee of any serious findings that need immediate attention, but will not initiate any diagnosis, treatment, or prescription.

PHYSICAL/VITAL MEASUREMENTS
The employee's height, weight, blood pressure and pulse rate will be measured and recorded.

LABORATORY
Prior to the date of testing, each employee will be provided with a container for the collection of a random urine specimen. This will be brought in on the morning of the employee's testing, at which time the phlebotomy technician will perform the necessary blood draw. The laboratory tests below will be performed by a CDC and CAP certified laboratory. The quality and accuracy of these tests are of the highest caliber, equal to that of a hospital environment.

ELECTROCARDIOGRAM
A resting 12-lead electrocardiogram with self interpretation will be recorded by a certified ECG technician. All abnormal ECGs will be over viewed by a board certified cardiologist for a nominal fee. As with all other tests, any findings requiring immediate attention will be directed to your company representative.

AUDIOMETRY
This hearing evaluation utilizes pure tone air conduction testing at six frequencies: 500, 1000, 2000, 3000, 4000, 6000 Hz. Testing is conducted in sound attenuating rooms under the direction of a certified audiology technician. The instruments are computer controlled to help eliminate variables, and are calibrated to the ANSI-69 standards. With previous baseline information, a separate computer analysis of all audiograms can be generated with special emphasis on the following categories: OSHA Standard Threshold Shift Audiologically Significant Noise and/or Medical Shifts Baseline Audiograms Demonstrating Hearing Loss Revised Baselines Due To Improvement in Hearing

SPIROMETRY (PULMONARY FUNCTION TEST)
The spirometer is attached directly to a microprocessor for immediate measurement and verification. Under the direction of a certified pulmonary technician. the employee performs three deep-breath efforts with the best measurement being recorded. These results are compared to the KNUDSON norms, as specified by the N1OSH Cotton Dust Standards and include the measurements and ratios of:
  • Peak Expired Flow Rate (PEFR) Forced Vital Capacity (FVC)
  • Forced Expired Volume at I second (FEV-1)
  • Ratio of (FEV- I/FVC) Predicted


  • CHEST X-RAY
    A 14xl7 view of the chest will be taken for each employee. The films will be processed on board the mobile clinic before the employee leaves. If necessary, any repeat x-rays can be taken while the employee is still present. All films will be interpreted by a board certified radiologist.

    VISION
    Vision testing is performed by a technician trained in the use of a Titmus vision screening instrument. The tests will consist of the following: Visual Acuity--both eyes together and individually Phoria Laterial and Vertical Color Perception Depth Perception

    HEALTH PROFILE REPORT
    All data collected in the health testing program will be computer processed and printed on the health profile report. The report is formatted so that up to five years of data for each test can be sequentially displayed across the page, providing an easy comparison of previous test data. The back page of the report contains a problem summary section designed for quick review by a physician of all the test findings which will be specially noted. This data is identified in the body of the report by one or two asterisks, or a L (Low) or H (High) designation. Mobile Health Testing, Inc. will provide two health profile reports per employee, along with the collected data consisting of EGG, audiogram. x-ray, PFT, and lab report.
